The Keto Weight-Loss Timeline: Realistic Expectations
On keto, expect water loss in week one, a stall with low energy in weeks two to four, and slower, steadier fat loss from about month two, so give it 60 days before you judge it.

The timeline at a glance
| Period | What usually happens | What he says to do |
|---|---|---|
| Week 1 | Fast drop, mostly water | Do not count it as fat |
| Weeks 2–4 | Stall, low energy, gut upset | Keep going; do not weigh |
| Month 2 on | Fat loss, about 0.5–2 lb a week in his estimate | Weigh weekly or every two weeks |
| Day 60 | First fair point to judge | Decide whether it is working |
The schedule is his, repeated across several videos (video uDw65ERtsA0; video UygKGoP_BtA; video cTGYIFOTPl8). He frames it as what to expect, not a promise of pounds per week (video uDw65ERtsA0).
Week 1: water
Your body uses up stored carbs (glycogen), and the water stored with them leaves too. Lower insulin also lets more sodium, and water, leave through the kidneys.
KetoDick on this: he puts the week-one drop at about three pounds of water, and 7–10 pounds for heavier people (video uDw65ERtsA0). He says weight quoted in the first 14 days is mostly water (video cTGYIFOTPl8).
Full explanation: water weight on keto.
Weeks 2 to 4: the stall
After the water is gone, the scale often stops. This is also when many people feel worst: tired, hungry, with gas or constipation.
Why it happens, in his account:
- Your body has run out of stored carbs but is not yet good at burning fat. He calls this the gap before fat adaptation (video uDw65ERtsA0).
- Gut bacteria are adjusting to fewer carbs (video uDw65ERtsA0).
- You are still eating to fullness, as month one calls for, so the calorie deficit may be small.
KetoDick on this: he calls weeks two and three "a slog," from the video, and says most people plateau then (video uDw65ERtsA0). In his 2018 basics video he says little weight change for two to four weeks after the water is normal (video UygKGoP_BtA).
A stall this early is not a sign the diet failed. See the week-three stall and stall or fluctuation?.
Month 2 onward: fat loss
His estimates for the fat-loss phase vary a little by video:
| Video | When it starts | Pace |
|---|---|---|
| uDw65ERtsA0 (2017) | About week four (weeks 3–6) | About 0.5–3 lb a week, depending on size and prior diet |
| cTGYIFOTPl8 (2017) | Days 30–60 | Not given |
| UygKGoP_BtA (2018) | Month two | About 0.5–2 lb a week |
In the 2017 video he ties the pace to body size and prior diet (video uDw65ERtsA0).
KetoDick on this: he says calories can come down naturally in month two, and to keep protein and fibre up as they do (video UygKGoP_BtA). He also says weight gained gradually over years will not come off in a month (video cTGYIFOTPl8; video Ewo-C_Cc4hU).
What the trials show over a year
Trial averages are smaller than a steady 0.5–2 lb a week would add up to. Individual results vary widely around these averages.
| Trial | People | Length | Average loss |
|---|---|---|---|
| Samaha 2003 | 132 adults with severe obesity | 6 months | Low-carb 5.8 kg vs 1.9 kg on the comparison diet (NEJM) |
| Foster 2003 | 63 adults | 12 months | Low-carb ahead at 3 and 6 months; not significantly different at 12 (NEJM) |
| DIETFITS 2018 | 609 adults | 12 months | Healthy low-carb 6.0 kg vs healthy low-fat 5.3 kg, not significantly different (JAMA) |
| DIRECT 2008 | 322 adults | 2 years | Low-carb 4.7 kg, Mediterranean 4.4 kg, low-fat 2.9 kg (NEJM) |
| Bueno 2013 (13 trials) | Pooled | 12 months or more | Very-low-carb keto 0.91 kg more than low-fat (Br J Nutr) |
Two notes. DIETFITS used a moderate low-carb diet, not a ketogenic one. And keeping people on any diet for a year is hard; Foster 2003 reported low retention in both groups.
His figure and the trial averages differ. Both are stated here: his is an estimate of what to expect, and the trials report averages across everyone enrolled. See keto vs low-fat and DIETFITS explained.
Keto also does not appear to speed fat loss through a metabolic edge. In a ward study of 17 men switched to a ketogenic diet at the same calories, energy expenditure rose by 57 kcal a day, and body-fat loss slowed after the switch (Hall et al. 2016). See the metabolic advantage question.
Why 60 days
Month one is a roller coaster of water loss, a stall and adaptation. Judging the diet at day 10 or day 20 means judging it on the hardest, least typical weeks.
KetoDick on this: he says to give the diet a full 60 days before deciding (video uDw65ERtsA0). He calls early weight not a "key performance indicator," from the video (video xyEZL0HuejI).
How to measure progress
| Measure | How often | Why |
|---|---|---|
| Photos | Day 1, then monthly | Shows changes the scale misses |
| Tape or string around waist and hips | Every 2–4 weeks | Fat loss with little scale change shows here |
| Scale | Skip days 1–30; then weekly or every two weeks | Less noise from daily water swings |
| Clothes fit | Ongoing | Simple and free |
KetoDick on this: he prefers photos and tape or string measurements to the scale, and says the scale exaggerates slips (video UygKGoP_BtA). He posted his own five-pound overnight gain to show that daily swings are normal (video f9P8BPWHc4U). See non-scale progress, losing inches, not weight and how often to weigh yourself.
Comparing yourself to posted results is a common trap. KetoDick on this: he says social media posts are cherry-picked and that comparing to them can do more harm than the scale (video dNUvzMl-OEU). See comparing on social media.
What can change your timeline
- Starting size. He says heavier people see larger early drops (video uDw65ERtsA0).
- Prior diet. He says the fat-loss pace depends on what you ate before, as well as on size (video uDw65ERtsA0).
- Hidden carbs and calories. See not losing weight on keto? and the plateaus chapter.
- Medication changes. Some diabetes, blood pressure and other medicines affect weight and fluid. Talk to your doctor if you take them, before you start and as your weight changes.
If you find yourself cutting harder to "catch up" with a timeline, stop and reset. A history of disordered eating is a reason to plan any weight goal with your doctor.
