Keto made easy

Moving From Weight Loss to Maintenance

When you reach your goal, you add food back slowly, usually fat first and then carbs, until your weight holds steady. Keep tracking for a few weeks while you find that level.

On this page:

What changes at maintenance

During weight loss you eat less energy than you burn. At maintenance you eat about the same as you burn. That means eating more than you did while losing.

The goal also changes. You are no longer chasing a lower number. You are holding a range and keeping the habits that got you there. See energy balance.

The three phases

Phase Typical length Fat in the diet What the body is doing
Introductory Weeks to a couple of months Fairly unlimited, to manage hunger Adapting to burn fat and ketones
Weight loss Months Comes down; body fat fills the gap Running a deficit, often without forced restriction
Maintenance Ongoing Comes back up Eating at about energy balance

KetoDick on this: he splits keto into these three phases. He gives the introductory phase about 2–4 weeks for thinner people and 4–8 for heavier people. At maintenance, he says, dietary fat has to come back up, to roughly 70–75% of the diet, with more total food (video NUJNFG6lLWw). He also says fat is the variable across all three: eat to fullness at first, it falls during weight loss, and rises again at maintenance (video UygKGoP_BtA).

Step by step: the transition

  1. Pick a goal range, not a single number. Daily weight swings are normal. See stall vs fluctuation.
  2. Track for a few weeks. Log food again, even if you stopped. See tracking.
  3. Add a small amount of food at a time. Increase by one food or one serving, then hold for a week or two.
  4. Watch the weekly average, not the daily number. If the average keeps falling, add a little more. If it rises steadily, step back.
  5. Keep protein steady. Protein helps you stay full and helps keep muscle. A review found it supports weight maintenance through satiety and by helping retain fat-free mass (Westerterp-Plantenga et al., 2009).
  6. Stop tracking once your weight holds for several weeks. Check in again if it starts to drift.

KetoDick on this: he suggests setting a food app to a maintenance calorie goal even during weight loss, so the first step is simply not gaining any more (video DoSUgIr2rNo). He suggests weekly averages over daily targets, and tracking when you start and when you stall (video UygKGoP_BtA). The post MyFitnessPal for keto covers setup.

Option A: stay keto, eat more fat

Some people stay low-carb and raise fat to reach maintenance energy.

Add Examples
Cooking fat Butter or oil on vegetables
Fattier proteins Thighs instead of breasts, fattier cuts of meat
Sauces Pan sauces and dressings
Whole-food fats Avocado, nuts, cheese, olives

KetoDick on this: in his meal-prep demo, he says to thicken the butter sauce on lean pork chops for maintenance. He asks viewers to write three to five go-to meals with macros, at least one of them for maintenance (video hHFu6L-yedE). The post eat fat to satiety covers the hunger side. See also how much fat.

Option B: add carbs back

Other people move to a less strict low-carb diet. If you choose this:

  1. Add carbs in small steps. One extra serving of a whole-food carb, such as fruit, beans or a starchy vegetable, then hold.
  2. Expect a quick gain on the scale. Carbs bring back stored glycogen and the water bound to it. That is water, not fat. See water weight.
  3. Notice hunger and cravings. If a food brings cravings back, it may be one you keep at a smaller dose.
  4. Find the level you can hold. That point, where weight is stable and hunger is calm, is your working carb level. See how many carbs.

KetoDick on this: he calls keto "a way of eating, not necessarily a way of life" (from the video). He says you can be low-carb for life, but he is unsure about very low carb for life. He suggests lower background insulin after weight loss may let some people return to more normal eating (video uXzscHudgt0). What to expect on the scale

What you see Likely cause What to do
A quick jump in the first week of more carbs Glycogen and water (he puts the glycogen-plus-water total at about 4.4 lb, video UygKGoP_BtA) Hold steady; watch the weekly trend
Weight still falling Not yet eating enough Add a little more food
Slow, steady rise over several weeks Eating above maintenance Step back one change
Hunger and cravings return A food may be a trigger Lower that food's dose

Regain often comes back slowly, as old routines return. The post regaining weight after keto covers why, and habits that keep weight off covers how people prevent it.

Medicines and the transition

If your doctor lowered your diabetes or blood pressure medicines when you started keto, tell them before you add carbs back. Your blood sugar and blood pressure may change again, and your doses may need another look. See keto and medications.
